Disciplinary History Requests

This page and form is for internal TCNJ Disciplinary History Requests ONLY.

The Dean of Students Office is looking to streamline the process for internal College background checks. It is our hope that this will create a consistent process that will allow for efficiency, as well as more transparency with students. In order to help us better meet your needs, please review/follow the steps below prior to submitting your request:

 

Disciplinary History Request Form

 

FOR ALL REQUESTS, PLEASE NOTE

We ask that you give us a minimum 5-7 business days to perform the checks.

The information that will be released will only include if a student is on a current/active disciplinary status or has outstanding incomplete sanctions.

  • Requests associated with the NCAA Board of Governors Sexual Violence Policy requirements are excluded from this exception and the existence of any pending cases will also be disclosed.

 

For offices that DO have a job/position/leadership application

We strongly encourage you to include the following language in your application if it does not already exist:

“In accordance with the Family Educational Rights and Privacy Act of 1974 (FERPA) (20 U.S.C. § 1232g; 34 CFR Part 99), the College may disclose information from the education records of a student provided the College has on file written consent from the student. This authorization will allow access to only authorized recipient(s) for the following type of education record for the duration of your employment and/or participation in the activity sponsored by [insert department/office name]. 

By signing below, I give consent for the Office of Student Conduct and Off-Campus Services and Office of Title IX & Sexual Misconduct to disclose my disciplinary record to [insert department/office name]. I understand that if I have any questions about the information that may be released, I may contact the Office of Student Conduct & Off-Campus Services and the Office of Title IX & Sexual Misconduct at conduct@tcnj.edu and titleix@tcnj.edu, respectively. 

I understand that I may withdraw permission to release my educational record by submitting a request in writing to conduct@tcnj.edu and/or titleix@tcnj.edu.”

 

Once you have collected all of the signed applications (that include disciplinary releases), you should:

  • Combine all of them into one singular PDF document.
  • Create an Excel sheet that contains the following information for each student:
    • Last name
    • First name
    • PAWS ID
    • TCNJ email address
  • Upload the PDF of the applications/releases AND the Excel spreadsheet containing the students’ information to this request form.

NOTE: Please do NOT share the list of students or access to the signed applications or authorization forms with us via Google Sheets or Drive – these should be submitted in the manner described above.

For offices that do NOT have a job/position/leadership application

Have each student fill out our Authorization to Disclose Information Form and return the completed form to your office.

Once you have received the completed authorization forms:

  • Combine all of them into one singular PDF document.
  • Create an Excel sheet that contains the following information for each student:
    • Last name
    • First name
    • PAWS ID
    • TCNJ email address
  • Upload the PDF of the applications/releases AND the Excel spreadsheet containing the students’ information to this request form.
NOTE: Please do NOT share the list of students or access to the signed applications or authorization forms with us via Google Sheets or Drive – these should be submitted in the manner described above.
